A Children’s Product Certificate is a specific document. Miss a required element and it does not count. CPC ShadowOps’s form maps to all seven:

Does Amazon really require a CPC?
Yes, for children’s products: Amazon asks for the certificate and the underlying test report from a CPSC-accepted lab, and listings can be suppressed without them. The same requirement comes from CPSIA itself, so it applies well beyond Amazon.
Can I just use a Word template?
You can, and many sellers do: that is exactly why templates get sellers in trouble. Templates go stale, omit newly cited rules, get filled in with the wrong factory details, and the finished certificates disappear into email threads.
